Advancing health equity in underserved populations requires an innovative, community-centered approach that leverages artificial intelligence (AI) and digital health tools. This white paper proposes a framework for AI-driven smart health equity - a system of community-based biomedical monitoring and preventive analytics designed to bridge gaps in preventive care. The strategic vision is to empower underserved communities with continuous health monitoring, personalized early warnings, and culturally tailored interventions delivered through community health networks. By integrating wearable biomedical sensors, AI analytics, and human-centered design, the framework enables early detection of health issues and proactive preventive care outside traditional clinical settings. The overarching goal is to reduce health disparities by ensuring that all populations benefit equitably from advancements in digital health.
