Mission

The mission of Longfellow/Seward Healthy Seniors Program is to enable community seniors to live healthy, independent and socially connected lives. Our service area includes five south Minneapolis neighborhoods: Longfellow, Seward, Cooper, Howe, and Hiawatha. The area is bounded by the Mississippi River on the east; Hwy. 55 on the west; Interstate 94 on the north; and Minnehaha Falls Park on the south. We also provide outreach to three adjoining neighborhoods including Cedar Riverside, Ventura Village and Philips West to provide outreach to the BIPOC community and the Somali refugee population in particular. We're a Living at Home/Block Nurse Program and a member of the Living at Home Network.
