THE CENTER IS AN INNOVATIVE MODEL OF COMMUNITY-BASED CARE FORMED TO IMPROVE THE HEALTH STATUS OF INDIVIDUALS WITHIN ITS SERVICE AREAS. THE CENTER OFFERS HIGH QUALITY, COST EFFECTIVE, COMPREHENSIVE PRIMARY AND PREVENTATIVE HEALTH CARE SERVICES FOR PEOPLE OF ALL AGES, AND EXTENDS ITS EDUCATIONAL AND HEALTH SCREENING SERVICES IN A VARIETY OF SETTINGS THROUGHOUT THE COMMUNITY. THE CENTER HAS BEEN DESIGNATED AS A FEDERALLY QUALIFIED HEALTH CENTER AND HAS ESTABLISHED SITES LOCATED IN ASBURY PARK, KEYPORT, RED BANK AND FREEHOLD, NEW JERSEY.
